Sports Physicals

For participation in any TFMS sport, the following items need to be completed prior to the first official practice:

1. Comprehensive Athletic Registration Form (found in the Genesis - Parent Portal)

2. Pre-participation Physical Evaluation Medical Eligibility Form (linked below - submitted to TFMS School Nurse via email or in-person)

3. Health History Update Form (found in the Genesis - Parent Portal)

All forms need to be completed prior to the first official practice. No child will be able to participate without all forms completed.
